What's on the card

Front — Wong-Baker FACES scale (ages 3–8):

• Six faces from "no hurt" to "hurts worst", with 0–10 ratings

Reverse — FLACC behavioural scale (under 3 years):

• Face, Legs, Activity, Cry and Consolability, each scored 0–2
• Total score guidance: 0 none, 1–3 mild, 4–6 moderate, 7–10 severe
